Specialty Support Overview

CHAS HIV Program (CHIV)

The CHIV program provides early intervention and health services for low-income, underinsured and uninsured people living with HIV/AIDS. Primary Care Services include:

  • Medical evaluations & testing
  • Oral health care
  • Nutritional counseling
  • Outpatient behavioral health counseling
  • Referrals to specialty care and other community resource as needed

Chronic Disease Management

CHAS provides collaborative case management for patients living with depression, asthma and diabetes. Ask your provider how this service works in decreasing or delaying potential complications of chronic disease and generally improving your quality of life.
